LEE COUNTY — Lee County Health Department now has an appointment request form for all residents who are eligible for COVID-19 vaccination under Phase 1B.
As a reminder, Phase 1B is expanding to residents age 65 and older, and frontline essential workers.
The form is available on the front page of the county health department’s official website at www.lchd.com.
Please do not complete this form if you have already called LCHD to request an appointment.
If you have provided your information to a staff member via phone, Facebook Messenger, or email, then you are already on the call list and will be contacted for scheduling as soon as possible. If you have left the health department a voicemail to request scheduling, a staff member will return your call as soon as possible to gather more information and proceed scheduling.
Upon completing the form, you will automatically be added to the call list, and an LCHD staff member will call you for scheduling at their soonest availability.
